Sore Throat and Tonsillitis: When Do You Need Antibiotics?

Most sore throats are viral and do not improve with antibiotics. When the clinical picture suggests streptococcal infection, testing and clinician assessment guide the antibiotic decision.

What should you know?

Cough, runny nose and hoarseness can support a viral cause. Without clear viral features, appearance alone cannot reliably distinguish strep. Children may need a throat culture after a negative rapid test, depending on age and circumstances. Follow the prescribed course for confirmed infection and do not share leftover antibiotics. Resistance develops in bacteria, not the human body. Fluids and suitable pain relief may ease symptoms.

Assessment and next steps

Assessment covers the mouth, throat and neck; endoscopy or tests are selected when needed. Antibiotics, acid-suppressing medicines and surgery require an appropriate diagnosis. Avoiding smoke and maintaining suitable hydration may reduce irritation.

Care and when to seek help

Difficulty breathing or inability to swallow saliva is an emergency. Persistent one-sided pain, a neck lump, voice changes or unexplained weight loss should be assessed promptly.
